As with many other symbiotes throughout Marvel's history, this one is illustrated by Spider Man artist extraordinaire, Mark Bagley! Page 13 of this issue is the "gestation" page, where we see the symbiote begin to envelop itself around Eddie Brock. The page that follows that one, page 14, is the birth page featuring a giant splash page image of Sleeper and was also used as a cover.
